Waiver and Release of Liability
I, the undersigned, as the parent/guardian of the above-named student, hereby grant permission for my child to participate in the activities and programs of Iqra Academy. I understand that these activities may include but are not limited to educational classes, physical activities, field trips, and other related events.
I acknowledge that participation in these activities involves inherent risks, including the risk of injury. I understand that the staff and volunteers of Iqra Academy will take all reasonable precautions to ensure the safety of my child. However, I acknowledge that accidents and injuries can happen, and I agree to assume all risks associated with my child’s participation in the school activities.
In consideration of my child being allowed to participate in the activities of Iqra Academy, I hereby release, waive, and discharge Iqra Academy, its directors, officers, employees, volunteers, and agents from any and all claims, liabilities, damages, or actions for any injury, illness, or harm that my child may sustain while participating in the school activities.
Medical Consent
In the event of an emergency, I authorize the staff and volunteers of Iqra Academy to seek medical treatment for my child. I understand that every effort will be made to contact me or the emergency contact provided above in such an event. I agree to bear any costs incurred for the necessary medical treatment.
Code of Conduct
I understand that my child is expected to follow the rules and guidelines set forth by Iqra Academy. I agree to support the school’s efforts to maintain a safe and respectful environment. I acknowledge that failure to adhere to these rules may result in disciplinary action, including but not limited to suspension or expulsion from the school.
Photo and Video Release
I grant permission for Iqra Academy to take photographs and videos of my child during school activities. I understand that these images may be used for promotional purposes, including on the school’s website, social media, and other marketing materials. If I do not wish for my child’s images to be used, I will submit a written request to the school administration.
Acknowledgment and Agreement
By signing this waiver form, I acknowledge that I have read and understood the terms and conditions outlined above. I agree to the terms and conditions and consent to my child’s participation in the Iqra Academy activities.
